As per the numerous requests we received last evening, below please find a copy of the NJ Community Theatre Cruise Perry Award Reservation Form (with $100 per person discount). Simply copy and paste this form into a WORD document, fill out completely and fax to us by 12 midnight on Sunday September 28th to receive the special NJACT Perry Award $100 per person discount.
